Observations

Observations entering and editing is done via double-click on the inspection record in the main screen. Than click on the observations bar.

For entry of new observations click the Insert Button.


Question tree

Observations can be entered and edited via the questions tree. If desired ,the displayed questions can be filtered via the Observations button. Next to all observations only those which got a negative answer, so called deficiencies can be displayed. Further open observations or overdue observations can be selected via this filter.


Comments

Inspector Comment & Owner Comment

If a question is selected in the question tree, the related informations are displayed.

There are 2 tabulators. One for the Comments and one for the Actions. The information can be entered by typing, copy and paste or drag and drop. The comments tabulator shows the Inspector Comment and the Owner Comment.
After entering information into these boxes the information must be saved! Click on SAVE Otherwise the information is lost!


Actions

Root Cause Investigation

Clicking the Action tabulator changes the view to 3 other sections: Root Cause, Immediate Action and Suggested Corrective Action. In case of a more severe deficiency that requires a Root Cause Analysis these boxes should be used to capture the information.
After entering information into these boxes the information must be saved! Click on SAVE Otherwise the information is lost!


Repetitions

Vessel repetions and fleet repetitions are identified

Repetitive observations are observations that occurred more than once on the same ship under the same inspection questionnaire. The database indicates the occurrence of a repetition, by displaying an additional tabulator called Repetition Warning or Fleet Repetition next to the Comments and Action tabulator.


If this tabulator is clicked the repetitions are listed by showing on line with ship name, Inspection Date and Inspection Port for each repetition.
The red line indicates a vessel repetition: The same observation occurred already on the same ship.
The grey line indicates a fleet repetition: The same observation recurred within the fleet.
The line indicates the ship name, the inspection date and the port of the recurrence.

Clicking the cross in the line line opens a field displaying the inspector and owner comment belonging to the repetition. Underneath the owner comment is a INSERT button.

Clicking this button copies the previous owner comment into the owner comments box of the selected question. A message box will pop up asking for confirmation. If the owner comment box has already an entry a warning is displayed asking whether you want to overwrite the existing comment.

 


Guidance Notes

Question guidance is displayed if applicable

Many industry questionnaires have guidance notes explaining the rationale of the question in detail. If such guidance note is available for the question, a tabulator with the text Guidance Note opens.
Clicking the tabulator displays the guidance note.

Attached Files

Documents can be attached to each observation

By opening the tabulator Attached Files all kinds of electronic documents can be attached to each observation. For more details see section Document Management

Analysis

Analysis attributes Severity, Cause, Responsibility and General References can be recorded. Details can be found in section Analysis.
